Declared Value Liability: NLVS assumes no liability as to the declared value on units of semen. NLVS will handle, store, and ship all semen with the utmost care and concern but will be held harmless for any and all loss or damage that may occur when semen is handled, stored, or shipped. It is the responsibility of the owner of the semen to maintain adequate insurance to cover loss or damage as described above.

New Life Veterinary Services — Surgical & Procedural Waiver and Consent

By consenting you authorize recommended procedures, diagnostics, and anesthesia.

All procedures carry risks (infection, complications, death); outcomes aren’t guaranteed.

You release NLVS from liability for routine complications, except for gross negligence or willful misconduct.

Vets may perform urgent additional treatments if needed; we’ll try to contact you. Extra charges may apply.

Follow pre/post-op instructions and pay for all services rendered.

We may use de-identified photos/records for care or education; promotional use requires separate consent.
